Ankle Pain

Ankle pain can often occur due to a number of conditions including ankle fracture, arthritis, instability, tendonitis, and gout.  Symptoms that accompany ankle pain include swelling, stiffness, redness and warmth in the affected area.  Pain associated with the ankle is typically described as a dull and intense ache that is felt during ankle motion or when bearing weight.

While ankle pain treatment can vary depending on the severity of the condition, initial treatment often involves rest, ice, elevation and immobilization.  Nonsteroidal anti-inflammatory drugs (NSAIDs), such as ibuprofen, may also be prescribed.  Other possible treatments include physical therapy and cortisone injection.  Your podiatrist can determine the method of treatment that is most appropriate for you and your condition.

There can be many different causes of ankle pain, ranging from an ankle sprain or fracture to chronic conditions such as arthritis. The degree of the ankle pain can vary and it can come on suddenly or slowly over time. The ankle joint consists of bones, muscles, cartilage, and tissues called ligaments and tendons. Any injury or disease that were to affect these structures, could very well lead to ankle pain. Some causes of ankle pain include sprains, tendonitis, arthritis and a bruise or break of the bone. It is important to know that you should visit a doctor if there is an inability to bend the ankle, swelling of the joint, signs of an infection, an inability to walk comfortably on the affected side and if you have ankle pain that occurs at night or while resting. Ankle pain can be diagnosed with an X-ray, medical exam or thorough physical examination. The RICE protocol is a 4-step process in which you may be able to treat your injury from the comfort of your own home. The first step is to rest. This will allow for the joint to relax and for any inflammation to subside. The next step is to ice. Ice should be applied at least 3 times a day for 15 to 20 minutes. This will allow for any swelling to go down and it will also soothe the pain. The third step is to add compression. Compression bandages, such as an ACE wrap will help support and immobilize the ankle joint. Be sure not to add too much compression as this can lead to numbness, tingling and swelling at the site of the pain. The final step is to elevate the ankle. Raising the ankle above the level of the heart, by way of propping your foot up on pillows, will help reduce any swelling for the first couple days after the injury has occurred. Flat Feet

Flatfoot is a condition that occurs when the arches on the foot are flattened, which allows the soles of the feet to touch the floor. Flatfoot is a common condition and it is usually painless.

Throughout childhood, most people begin to develop arches in their feet, however, some do not. Those who do not develop arches are left with flatfoot. The pain associated with flat feet is usually at its worse when engaging in activity. Another symptom that may occur with those who have this condition is swelling along the inside of the ankle.

It is also possible to have flexible flatfoot. Flexible flatfoot occurs when the arch is visible while sitting or standing on the tiptoes, but it disappears when standing. People who have flexible flatfoot are often children and most outgrow it without any problems.

There are some risk factors that may make you more likely to develop flatfoot. Those who have diabetes and rheumatoid arthritis have an increased risk of flatfoot development. Other factors include aging and obesity.

Diagnosis for flat feet is usually done by a series of tests by your podiatrist. Your podiatrist will typically try an x-ray, CT scan, ultrasound, or MRI on the feet. Treatment is usually not necessary for flat foot unless it causes pain. However, therapy is often used for those who experience pain in their flat feet. Some other suggested treatment options are arch supports, stretching exercises, and supportive shoes.

An Overview of Flat Feet

Flat feet is a deformity that occurs when the arch of the foot collapses and comes into complete or near-complete contact with the ground. The condition occurs either at the time of birth or through injury and age. For the people that do develop flat feet, there are very few symptoms. They can include a limited range of motion, swelling along the inner ankle and arch of the foot, and unbearable pain when standing on your feet. The main cause of flat feet is excessive pronation of the foot. This means that the arch descends downward and inward as the foot strikes the ground. Due to the tendency for flat feet to over pronate, they are less able to absorb shock, placing persistent stress on the feet, ankles, and knees. If the inward tilt of the foot places further strain on the tendons and ligaments of the lower extremities, this can also result in Achilles tendinitis and chronic knee pain. Flat feet is most common in newborns and toddlers due to the arches on their feet not having developed yet. Imaging tests done to determine if someone has flat feet are X-rays, ultrasounds and MRIs. The treatments vary for children and adults. The most beneficial treatment for children would be walking barefoot during developmental years. For adults, the best forms of treatment are exercises that increase arch flexibility and strength such as foot gymnastics, a runner’s stretch, the yoga pose downward dog and therapeutic massages. What Are Flat Feet?

Flatfoot is a condition in which the arch of the foot is depressed and the sole of the foot is almost completely in contact with the ground. About 20-30% of the population generally has flat feet because their arches never formed during growth.

Conditions & Problems:

Having flat feet makes it difficult to run or walk because of the stress placed on the ankles.

Biomechanics in Podiatry

Podiatry is a branch of medicine that deals with the study, diagnosis, and treatment of foot and ankle conditions. There are various subdivisions in podiatry; biomechanics is one of them. Biomechanics is the way in which the bones, muscles, and joints of the feet and lower limb interact with each other.

Our feet play crucial roles in the way we move, and it is rare to have feet that are completely symmetrical. Common biomechanical issues include high or low arches or uneven leg heights. Excessive pronation often leads to fallen arches, or flat feet, and is a common cause of running injuries. People whose feet are over-pronated tend to have flexible and unstable feet. Pain is usually experienced during walking and running.

At times, people may be able to adapt to these abnormalities without any difficulties, but in other cases, these issues can cause a great deal of pain. This pain occurs because the joints, muscles, ligaments, and tendons are put under an excess amount of stress during movement. Common symptoms of biomechanical problems stemming from the feet include hip pain, knee pain, leg cramps, ankle pain, lower back pain, weak ankles, tripping, heel pain, Achilles pain, and shin splints.

Many biomechanical issues can be treated with orthotics. Orthotics are shoe insoles that are used to help control the way the foot operates.  They can provide relief from foot pain, heel pain, and knee pain. Depending on your specific case, you may need to purchase over-the-counter orthotics or custom orthotics to fit your feet. Your podiatrist will be able to prescribe the perfect orthotic for your feet to help you walk around with ease.

Gait is defined as the way we move our bodies from one point to another. This is usually done by either walking or running. Gait analysis is a method used to assess the way we walk or run to highlight biomechanical abnormalities. Gait analyses are a great way to take a detailed look at how you walk and how your foot moves while you walk. An examination of the feet will help your podiatrist understand why you are suffering pain in other parts of your body.

Biomechanical Problems of the Foot

As the foundation for your body, the feet provide both support and balance. It can be key to know the anatomy and function of the foot, as well as its shape, in order to be able to tell if it has become altered by a deformity or disease. Plantar Warts

Plantar warts are growths that typically appear on the heels or other weight-bearing areas of the feet. These warts are caused by the human papillomavirus (HPV). The virus enters the body through breaks in the skin, such as cuts, that are on the bottom of the feet. Plantar warts are more likely to affect children and teenagers, people with weakened immune systems, people who have a history with plantar warts, and people who walk barefoot in environments exposed to a wart-causing virus.

If you suspect you have plantar warts, you may have the following symptoms: pain or tenderness while walking, a lesion that interrupts the ridges in the skin of your foot, small fleshy lesions on the bottom of the foot, or a callus where a wart has grown inward over a well-defined spot on the skin.

HPV causes plantar warts to form and is very common. There are more than 100 kinds of the virus in existence. However, only a few of them cause warts on the feet. The other types of HPV are likely to cause warts on other parts of the body.

If you have plantar warts, your podiatrist may try different treatment methods depending on your specific case. Some treatments for plantar warts are peeling medicines (salicylic acid), freezing medicines (cryotherapy), or surgical procedures. Laser treatments and vaccines are also used to treat plantar warts.

What Are Plantar Warts?

Plantar warts are small growths that appear on the heels of the feet. Pressure may make the warts grow inward beneath a thick layer of skin called a callus. Plantar warts are caused by HPV. The HPV virus is able to enter the body through tiny cuts, breaks or other weak spots on the bottom of the feet. Some signs and symptoms of plantar warts include lesions on the bottom of the foot, callused skin where a wart has grown inward and pain or tenderness when walking or standing. Children and teenagers are considered to be the most at risk to get plantar warts.  Although plantar warts are not considered to be a health concern, you should still see a podiatrist in order to have the warts removed.

About Plantar Warts

Plantar warts are the result of HPV, or human papillomavirus, getting into open wounds on the feet. They are mostly found on the heels or balls of the feet.

While plantar warts are generally harmless, those experiencing excessive pain or those suffering from diabetes or a compromised immune system require immediate medical care. Plantar warts are easily diagnosed, usually through scraping off a bit of rough skin or by getting a biopsy.

Symptoms

  • Lesions on the bottom of your feet, usually rough and grainy
  • Hard or thick callused spots
  • Wart seeds, which are small clotted blood vessels that look like little black spots
  • Pain, discomfort, or tenderness of your feet when walking or standing

To help prevent developing plantar warts, avoid walking barefoot over abrasive surfaces that can cause cuts or wounds for HPV to get into. Sever's Disease

Sever’s disease, also known as calcaneal apophysitis is a common bone disorder that occurs during childhood. The disease is defined as an inflammation of the growth plate in the heel. When a child has a growth spurt, his heel bone grows faster than the muscles, tendons, and ligaments in his leg. This disease is a result of overuse. The people who are most likely to be affected by this disease are children who are in a growth spurt, especially boys who are from the ages of 5 to 13 years old. 60% of children with Sever’s disease have both heels involved.

Symptoms of this disease are heel pain that intensifies during running and jumping activities. The pain is typically localized to the posterior part of the heel. Symptoms may be severe, and they can easily interfere with daily activities. Children who play soccer, baseball, and basketball are more likely to develop Sever’s disease.

Your doctor will diagnose your child based on his or her symptoms, x-rays are generally not helpful in diagnosing this disease. Your doctor may examine both heels and ask your child questions about his or her activity level in sports. Your doctor may then use the squeeze test on your child’s heel to see if there is any pain. Nevertheless, some doctors might still use x-rays to rule out any other issues such as fractures, infections, and tumors.

Sever’s disease can be prevented by maintaining good flexibility while your child is growing. Another prevention method is to wear good-quality shoes that have firm support and a shock-absorbent sole. Sever’s disease can be treated by ceasing any activity that causes heel pain. You should apply ice to the injured heel for 20 minutes 3 times a day. Additionally, orthotics should be used for children who have high arches, flat feet, or bowed legs.

If you suspect your child has Sever’s disease, you should make an appointment with your podiatrist to have his or her foot examined. Your doctor may recommend nonsteroidal anti-inflammatory drugs (NSAIDs), such as ibuprofen or naproxen to relieve pain. In more severe cases, your child may need a cast to rest his or her heel. Fortunately, Sever’s disease does not cause long-term foot problems. After treatment, your child should start to feel better within two weeks to two months.
